Lost/Forgotten Aadhar

In order to get a new Aaadhar Card:
1) the first step is a visit to an authorized Aaadhar enrolment center and an appointment for the same can be booked at https://bookappointment.uidai.gov.in/
2) Upon visiting the center, the enrollment form has to be filled, having the demographic and biometric data captured, submitting the required documents and then collecting the acknowledgement slip which contains the enrollment ID. The documents that need to be carried are ones that carry proof of identity and proof of address. The list of documents which are accepted for different ages have been given in https://uidai.gov.in/images/commdoc/List_of_Supporting_Document_for_Aadhaar_Enrolment_and_Update.pdf .
HEAD OF FAMILY SYSTEM:
1) Even if the individual seeking to enroll does not have the valid documents, enrollment is still possible if the their name is present in the family entitlement document. In this case, it is necessary that the Head of the Family is already enrolled with the valid documents (proof of identity and proof of address). The Head of the family can then introduce the other members when they are enrolling for the Aadhar. Enrollment needs to be done only once and multiple requests may get rejected unless it is specially advised by the UIDAI (Unique Identification Authority of India). The process to apply for an Aadhar is free of cost. The waiting time for an applicant who is above the age of 18 can be around 90 days from the enrollment.
INTRODUCER SYSTEM:
1) The Introducer system: An introducer is an official person approved by the UIDAI or registrar who verifies your identity if you do not have standard proof of identity or address documents. The documents needed are the introducer name and Introducer’s Aadhaar number. The introducer must be at least 18 years old, have a clean record, and go to the enrolment center to vouch for you using their own Aadhaar and biometrics.
FOR CHILDREN BELOW 5:
1) Resident Indian/NRI child seeking enrolment need to visit an Aadhaar enrolment centre along with Mother and/or Father or Legal guardian submit a request in requisite form along with valid supporting documents. New Enrolment is Free of charge.
2) Documents needed:

Mandatory Demographic information (Name, Date of Birth, Gender, Address)
Optional demographic information (mobile number and email)
Biometric Information (Photo of child).
One of the parents/ guardian has to authenticate on behalf of the child and also give consent for enrolment of the minor by signing the enrolment form.
If the child is an NRI – valid Indian passport of the child is mandatory as Proof of Identity (PoI)

FOR CHILDREN BETWEEN 5 and 18:
One of the parents/ guardian has to give consent for enrolment of the minor by signing the enrolment form.
If the minor is an NRI – valid Indian passport of the child is mandatory as Proof of Identity (PoI)
If the minor is an Indian resident (not NRI) –
No document in the name of the minor: any valid Proof of Relationship document such as Birth certificate can be used for enrolment under Head of Family.
Minor has a document: use a valid Proof of Identity (PoI) and Proof of Address (PoA) document (like School ID card) in the child’s name for enrolment..

HOW TO TRACK APPLICATION:
1) The Aadhaar card application can be tracked on https://myaadhaarbeta.uidai.gov.in/CheckAadhaarStatus/en by entering your enrollment ID as mentioned on the acknowledgement slip.
